Submit a referral through the form below, or reach us by phone, fax, or email.

3

Coordinate the evaluation

Our office contacts the individual or caregiver to discuss the appropriate service and scheduling.

Referral Questions

Who can refer?

Physicians, therapists, psychiatrists, pediatricians, schools, and community professionals. Self-referrals are also welcome — a professional referral is not required.

Is insurance accepted?

Insurance may cover the ADHD diagnostic evaluation depending on individual benefits and billing requirements. We do not guarantee coverage or a specific patient payment. The $300 Autism Screening and the $1,500 Comprehensive Autism Evaluation are private pay.

Are evaluations available by telehealth?

Yes. Telehealth services are available to eligible individuals in Indiana, Michigan, New Hampshire, and Oregon, subject to licensure and clinical appropriateness.

What if there is prior testing?

Please include any prior testing or evaluation records with the referral. Prior information helps us determine the most appropriate next step.

Will I receive a report?

ADHD evaluations, Autism screenings, and Comprehensive Autism evaluations each include a written report. The screening report summarizes findings, clinical impressions, and recommendations; the comprehensive report integrates history, assessment findings, diagnostic reasoning, and individualized recommendations. Outside professionals receive results only when appropriately authorized by the individual or caregiver.

What if the diagnosis is uncertain?

An evaluation offers clarity to work from but does not guarantee a particular diagnosis. Sometimes ruling out a condition or identifying another explanation is equally valuable.
